All patients register at the registration desks located in the Emergency Department. When you arrive on campus, follow the signs to the Emergency Department. For your convenience, the registration desks are staffed 24 hours a day, 7 days a week.

As a reminder, it is important for all scheduled patients to arrive on time.

During the registration process our patients will be asked to sign the following:

• Consent of Treatment
• Authorization to bill the insurance provider

If you are a Medicare recipient, you will be required to sign the following:

• Medicare secondary questionnaire 
• Medicare rights statement
